Admission day can feel very stressful for residents as well as families. We strive to make the admissions process as seamless and comfortable as possible. It is helpful to bring the following information upon admission to ensure a smooth transition:

• ✓ HMO or gap filler insurance card
• ✓ Living Will and/or Power of Attorney
• ✓ Emergency contact information
• ✓ Picture ID
